How to Use Form 1095
If you or anyone else in your home had health insurance through the Marketplace in 2016, then you can expect to receive Form 1095-A in your mail no later than the middle of February. This form is the Health Insurance Marketplace Statement and you need to have an accurate copy of it prior to filing your tax returns.
Where to Find Your Statement Online
In addition to receiving this form in the mail, it will also be made available in your account on HealthCare.gov as early as mid-January. To gain access to it, you first need to log into your Heathcare.gov account.
Once you’re logged in, click the green "Start a new application or update an existing one" button.
Select "Go to my applications & coverage" at the bottom of the screen. Then, under "Your existing applications," select your 2016 application. Make sure you select your 2016 application and not the one for 2017.
On the left hand side of the screen you’ll see a menu. Select “Tax forms.” From here, you can download your Form 1095.
If for some reason you can’t find your Form 1095 in your account, contact the Marketplace Call Center for assistance.
What You Need to Do With Form 1095
When you receive your Form 1095, the first thing you should do is read the instructions listed on the back of the form. Then review the information included on the form for accuracy. This includes things like premiums paid, premium tax credits used, and a figure called the “second lowest cost Silver plan” (SLCSP). If anything is wrong with the information provided, you should contact the Marketplace Call Center as soon as possible.
What is the Second Lowest Cost Silver Plan?
The SLCSP figure is located in Part III, column B of your 1095-A. This number represents the number of months any household member had coverage under a Marketplace plan.
